Primary physicians are the first point of contact to the under-insured community and our organization is available to your patients as an affordable resource for diagnostic imaging studies.

Refer a Patient

If you would like to refer a patient to us you may : 1) Fax the referral to our scheduling fax at 855-345-5222 2) Call our nationwide scheduling hotline at 855-346-5152

To refer a patient, you can download a referral form and fax the completed referral to our scheduling fax at 855-345-5222. Alternatively, you may use a referral already written for the patient or any referral form of your choosing.

Referral Forms

Please ensure that any patients being referred list you as the referring physician so that reports are correctly forwarded to you.

Request Patient Literature

If you would like to provide your patients a low cost option for receiving imaging studies, please contact us via email at physicians@radiologyassist.com or give us a call at 855-346-5152. At no cost, we will send you literature about our program to hand out to your patients.

Our program is made possible by the relationships we have with collaborating imaging centers and radiologists. They are the backbone of our program and we are always looking to add to our network of collaborating imaging centers.
